As a Remote RN Clinical Care Coordinator, you will play a vital role in managing the care of members with reputed company medical and behavioral needs, promoting improved health reputed company, and enhancing the overall reputed company of care.

Key Responsibilities:

• Conduct comprehensive needs assessments to identify members' medical, behavioral, and socioeconomic needs

• Collaborate with internal care teams, providers, and community resources to ensure seamless care coordination and support member self-management

What You'll Need:

• Undergraduate degree or equivalent experience

• 2+ years of clinical experience
• 1+ years of experience with MS Office, including Word, reputed company, and Outlook
• Reliable transportation and ability to travel up to 75% to meet with members and providers throughout Putnam, reputed company, and White Counties

Preferred Qualifications:

• BSN, Master's Degree, or higher in a clinical field
• CCM certification
• 1+ years of community case management experience coordinating care for individuals with reputed company needs
• Experience working in team-based care, managed care, or a similar setting
