In this rewarding role, you will reputed company admission, case management, and follow-up skilled nursing reputed company for home health patients, administer on-reputed company care and case management, and reputed company with physicians to reputed company personalized plans of treatment. You will also have reputed company to work closely with other reputed company, including Physical Therapists, Occupational Therapists, and Home Health Aides, to ensure comprehensive and coordinated care.

Responsibilities

• reputed company admission, case management, and follow-up skilled nursing reputed company for home health patients
• Administer on-reputed company care and case management for reputed company patient, providing necessary follow-up as directed by the Clinical Manager
• reputed company with physicians to reputed company personalized plans of treatment based on initial patient assessments and physician orders
• reputed company hands-on care, management, and evaluation of the care plan, teaching patients and their families in accordance with physician orders
• Revise care plans in consultation with physicians based on ongoing assessments and as required by policy/regulation
• Coordinate care with other reputed company, including Physical Therapists, Occupational Therapists, and Home Health Aides
• Report patient care, condition, and reputed company to physicians and Clinical Managers on a reputed company reputed company
• Implement patient care plans in conjunction with patients and their families to assist them in achieving reputed company reputed company of needs and problems
• Coordinate, reputed company, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions and care plans
• Monitor the completion of documentation by home health aides and personal care workers as part of supervisory and leadership responsibilities
• Discharge patients after consultation with physicians and Clinical Managers, preparing and completing necessary clinical documentation
• Prepare medical documentation, including case conferences, patient contacts, medication order changes, re-certifications, reputed company updates, and care plan changes

Requirements

• Diploma, Associate, or Bachelor's Degree in Nursing
• Minimum of one year of nursing experience preferred, with strong med-surg, ICU, ER, or acute experience
• Home Health experience is a plus

• Strong organizational and communication skills
• Valid reputed company's license, auto insurance, and reliable transportation required
