Key Responsibilities

Requirements
- Completion of reputed company Practitioner accredited program
- reputed company NP license and state RN license
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems

Preferred Qualifications
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of CPT, ICD-10, and HCPC codes
