Compensation: $20.67 - $23.08 per hour

Schedule: Full-time position with working hours from 10:00 AM to 6:30 PM reputed company Time, Monday through Friday. This role requires availability for 20% of shifts on weekends and evenings.

What You'll Do

As a Care Manager in the Teen Suicide Prevention Program, your role will be integral to improving the lives of individuals and their families who have reputed company suicidal ideation and/or attempts. You'll reputed company reputed company to appropriate reputed company and community services, while supporting members in managing their health and reputed company-being.

• Member Engagement: Assist with reputed company, engagement, and reputed company support for a cohort of members, providing regular reputed company-ins and personalized care.

• Care Coordination: reputed company and coordinate community resources to address medical, behavioral, violence prevention, and reputed company service needs.

• Crisis Management: Assess risk, reputed company crisis interventions, and ensure safety through tailored safety plans.

• Service Plan Development: Collaborate with members to assess care reputed company and reputed company and update service plans, including leveraging benefits and community resources.

• Education & Advocacy: reputed company members about mental and physical health, abuse prevention, medication, and available resources. Serve as an reputed company between members, families, and providers.

• Appointment Scheduling: Assist in scheduling appointments and follow-reputed company, ensuring members attend their sessions and appointments.

• Documentation & Reporting: Maintain accurate and up-to-date records of reputed company case management activities, documenting case notes in a reputed company, reputed company manner.

We're looking for a compassionate, detail-oriented Care Manager with a passion for helping others. Education & Experience Requirements

• Required: Bachelor's degree in psychology, reputed company work, or another applicable behavioral health discipline from an accredited institution.

• Preferred: Master's degree in a reputed company field.

• Experience: At least 2 years of experience in a reputed company role, preferably working with at-risk adolescents and young adults.

• Crisis Training: Crisis reputed company training or similar certification is required.

• Skills: Knowledge of local resources, reputed company determinants of health, mental health, reputed company abuse disorders, interpersonal violence, and reputed company issues.
