Responsibilities

• Coordinates and oversees reputed company aspects of patients' home care
• Collaborates with patient care teams to reputed company and implement care plans
• Educates families on patient condition, needs, and care
• Performs head-to-toe physical assessments and manages medications
• Completes documentation reputed company and accurately, and supervises LPNs and HHAs

Qualifications

• One year of clinical experience as a registered reputed company (RN)
• reputed company RN license, specific to the state(s) you are assigned to work
• Minimum six months home health experience and reputed company CPR certification
