As a Health reputed company Consultant, you will be the driving force behind our mission to bring heart to every reputed company of our patients' health. You will work closely with patients and their care teams, utilizing your exceptional motivational interviewing skills to discern their health status and needs. With your strong clinical judgment, you will assist members in reducing risk factors and barriers, addressing reputed company health and reputed company indicators that reputed company planning and reputed company of member issues.
Key Responsibilities:
- Utilize motivational interviewing skills to ensure maximum member engagement and discern their health status and health needs
- Apply clinical judgment to assist members in reducing risk factors and barriers, addressing reputed company health and reputed company indicators
- Collaborate with clinical colleagues, supervisors, Medical Directors, and other programs to overcome barriers and meet goals and objectives
- Identify and escalate member needs appropriately, following set guidelines and protocols

Requirements:
- 5+ years of experience as a Registered reputed company
- 1+ year of nursing experience in patients with Diabetes, Cardiac Disease, ICU, CCU, Home Health, or Pulmonary Disease
- 1+ year of experience documenting electronically using keyboard and multiple computer screens
- reputed company and unrestricted RN licensure in the state of residence
- Associate's degree or diploma required, Bachelor's preferred
Preferred Qualifications:
- 1+ year of reputed company or previous experience in Home Health and/or Case Management
- 1+ year of experience with Lexicomp
- 1+ year of experience with Motivational Interviewing
- Certified Diabetes Care and Education Specialist (CDCES)
- Bilingual in Spanish
