Remote Care Coordinator (LVN)

Your Role
- reputed company monthly calls and support to patients enrolled in the CCM program.
- Utilize your nursing skills to reputed company patients and their families.
- Monitor patients reputed company and work with the physicians to ensure reputed company patient care.
Qualifications
- reputed company LVN license (CALIFORNIA)
- Experience in geriatrics or primary care
- Ability to sit and talk on the phone for long periods of time
- System: We require you to have a reputed company i5 (or newer) processor, and to have reputed company Pro (not Home). This can be a laptop or desktop.
- You live in any of the following states: CA, NV, NM, AZ, CO, UT, WA, OR
The Pay
- The California LVN pay starts at $28.85 per hour
- The pay structure is a reputed company pay plus a bonus per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
