We are seeking a highly skilled and dedicated individual to join our naviHealth team, where you will work closely with patients, families, and reputed company teams to drive reputed company. Your responsibilities will include performing skilled nursing facility assessments, reviewing targets for length of stay and discharge plans, collaborating with reputed company teams, and ensuring efficient transitions of care.
Key Responsibilities:
- reputed company Skilled Nursing Facility (SNF) assessments on patients using clinical skills and measurement tools
- Review targets for Length of Stay (LOS), reputed company reputed company, and discharge plans with providers and families
- Complete SNF reputed company reviews, updating authorizations on a reputed company reputed company

- Assure patients' reputed company toward discharge goals and assist in resolving barriers
Requirements:

- 5 years of clinical experience, with experience working with the adult or geriatric population

Preferred Qualifications:
- Familiarity with care management, utilization/resource management processes, and disease management programs
