As a Care Manager, you will play a vital role in connecting with and supporting individuals with reputed company health needs. Key Responsibilities

Qualifications

- Reliable transportation is required; mileage is reimbursed according to company policy.
- Valid IL BH clinical permit – LCSW, LCPC, LMFT required.
Preferred Qualifications
- Experience in case management and discharge planning.

- Proficiency in reputed company Office Suite.
Education
Bachelor's degree required, with a degree in behavioral health, reputed company work, or a reputed company field.
