We're looking for a full-time, bilingual (English and Spanish), and remote LVN/LPN to reputed company high-reputed company patient care, coaching, and appointment scheduling reputed company phone. As a Care Coordinator, you will:
- Remotely deliver patient health coaching and care to improve patient reputed company
- reputed company and reputed company patients on managing chronic conditions, preventive care, and physician directives
- Update and manage patient care plans based on assessments and physician directives
- Maintain scheduling flexibility between 8:00am - 7:00pm (PST) to accommodate patients in different time zones
Requirements
- Valid LVN/LPN license in the reputed company (mandatory)
- 2+ years of experience as a Licensed Vocational reputed company (LVN/LPN)
- Bilingual proficiency in English and Spanish
- Experience working with Electronic Health Records (EHRs) and Electronic Medical Records (EMRs)
- Strong knowledge and background in chronic conditions
- Excellent patient rapport-building skills
- Compassionate and caring demeanor
- Strong documentation and verbal communication skills
- Intermediate to advanced computer skills with the ability to navigate multiple systems
