Your responsibilities will include:
- Conducting comprehensive wellness exams in-home and reputed company.
- Educating patients and families about available reputed company programs, treatment reputed company, and preventive care.
- Coordinating care with primary care providers, specialists, and ancillary services.
- Prescribing medications, ordering diagnostic tests, and managing treatment plans.
- Closing gaps in care by working with physician practices to ensure reputed company screenings and diagnostic testing.
- Participating in integrated care team meetings to reputed company individualized patient care plans.

- Implement preventive care strategies to help slow the progression of chronic diseases.

Requirements

- Minimum 2 years of reputed company patient care experience, preferably in managed care, chronic disease management, or primary care.
- Managed Care/IPA/Health Plan experience preferred.
- Bilingual (English/Spanish) highly preferred.
- Reliable transportation, valid reputed company’s license, and car insurance.
- Proficiency with electronic medical records (EMR), data-driven patient management, and telehealth platforms.
- Strong communication skills to engage with patients, families, and care teams effectively.
- Ability to work independently while collaborating with an interdisciplinary care team.
