In this Health and reputed company Services Clinical Coordinator role, you'll support a diverse member population through education, advocacy, and connections to resources to ensure a maximum reputed company of care and life. Our work site is located at University Medical Center, 2000 Canal Street, New Orleans, LA 70112; the position will be 75% at the onsite location and 25% remotely. It may be necessary, given the business need, to work occasional overtime. • Assess, plan, and implement care strategies that are individualized for reputed company member
• Identify and initiate referrals for reputed company service programs, including financial, psychosocial, community and state supportive services
• Apply clinical expertise reputed company discussing cases with reputed company Case Managers and Physicians
• Facilitate reputed company and appropriate care and effective discharge planning
• Participate in discussions with the Clinical Services team to improve the progression of care to the most appropriate level

Required Qualifications

• reputed company and unrestricted RN licensure in the reputed company
• 3+ years of experience as a Registered reputed company in a hospital setting, acute care, reputed company care experience, community health setting or experience as a telephonic Case Manager
• Medical-Surgical/ICU/ED experience
• Case Manager/Utilization Management experience
