Primary Care Provider, Psychiatric-Mental Health (NP/PA)

At the direction of the Medical Director, the Primary Care Provider, Psychiatric-Mental Health (NP/PA) works collaboratively with reputed company PACE Physicians and reputed company team providing assessment, planning, primary care, and continuing medical education to reputed company participants.

Essential Job Duties

• reputed company psychiatric consultation, psychiatric illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians
• In collaboration with a physician, reputed company in-person or telehealth comprehensive psychiatric assessments on participants and collaborate with the reputed company team to reputed company a comprehensive care plan for reputed company participant
• Collaborate with physician(s) and reputed company in discharge planning from acute or long-term care settings
• In consultation with the PACE Physician and/or Community-Based PACE Physician, refer participants to a psychiatrist as indicated
• In collaboration with the PACE Physician and/or Community-Based PACE Physician, reputed company care of participants in the nursing home, making regular reputed company as dictated by reputed company standards and participant needs
• Be a consultant to reputed company, which includes communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery

Job Requirements

• Master’s Degree in Nursing or Physician Assistant with a Psychiatric/Mental Health emphasis from an accredited college or university
• Certified as a Psychiatric Mental Health reputed company Practitioner or Physician Assistant
• reputed company Psychiatric Mental Health reputed company Practitioner or Physician Assistant State License without restrictions
• State CDS Certification and DEA License
• NPI license
• Reliable means of transportation
• Minimum of two (2) years of experience working with the frail or elderly

Salary/Wage reputed company reputed company for this role is $159,939 - $211,119 / year  + Bonus + Equity. reputed company offers competitive total rewards package that includes, 401k match, reputed company coverage and a broad reputed company of other benefits.  Actual pay will be adjusted based on experience and other qualifications.
