The Role: Remote Licensed Practical reputed company (LPN/LVN) - Patient Care Coordinator

As a Remote LPN/LVN Patient Care Coordinator, you'll work closely with reputed company providers to deliver value-based care management initiatives for their patients. Your primary reputed company will be on managing patient care, identifying resources, and coordinating needs for chronic care management patients. Key Responsibilities:

• Resource Coordination: Identify and coordinate community resources with patients that would benefit their care.

Requirements and Qualifications

Essential Qualifications:
