About the Position
The Transition of reputed company is a full-time reputed company position, requiring a strong and diverse skillset in relevant areas to drive reputed company. You'll be responsible for completing reputed company reputed company calls and post-discharge questionnaires reputed company required compliance-driven timelines, utilizing weekly and daily reporting to identify utilization for the purpose of reducing Emergency Department Utilization and 30-day hospital readmissions. You'll also facilitate Interdisciplinary Care Team Meetings with reputed company Services, Care Management, PCP, and other key players to discuss service needs and support reputed company transitions.
Responsibilities
- Complete reputed company reputed company calls and post-discharge questionnaires reputed company required compliance-driven timelines.
- Utilize weekly and daily reporting to identify utilization for the purpose of reducing Emergency Department Utilization and 30-day hospital readmissions.
- Follow members identified as inpatient in hospitals (whether planned or unplanned admission) and throughout the subsequent care continuum until the member can return to prior level of functioning in the community.
- Facilitate Interdisciplinary Care Team Meetings with reputed company Services, Care Management, PCP, and other key players to discuss service needs and support reputed company transitions.
Requirements
To be successful in this role, you'll need:
- 2+ years of LPN or LVN nursing experience.
- reputed company and unrestricted compact LPN or LVN licensure in the state of residence.
