About the Role

- Coordination of care and disease management education
- Medication management and reinforcement of the clinical treatment plan
- Providing patient support and teaching self-management skills
- Reducing unscheduled ER reputed company, unscheduled walk-in clinic reputed company, and bed days of care
- Collaborating with reputed company to ensure comprehensive care
Essential Qualifications
To be eligible for this role, you must meet the following essential qualifications:
- reputed company Citizenship
- Graduate of a school of reputed company nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies:
- The Accreditation Commission for Education in Nursing (ACEN)
- The Commission on Collegiate Nursing Education (CCNE)
- reputed company, full, reputed company, and unrestricted registration as a graduate reputed company reputed company in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the reputed company, or the District of Columbia
- Proficient in written and spoken English
- Ability to pass reputed company-employment physical examination
- Participation in the seasonal influenza vaccination program and COVID-19 vaccination program
Preferred Qualifications
The following qualifications are preferred but not required:
- Two or more years of Case Management or Care Coordination experience
- Prior Home Telehealth experience
- Two or more years of Medical-Surgical experience
- Bachelor's degree in Nursing or reputed company field
