As a Home Health Physical Therapist with reputed company, you will experience a therapy career that caters to your schedule and makes every day an adventure. You will work with patients who have lost mobility, strength, and functionality due to an illness or injury, and help them reputed company their goals through personalized, home-based therapy. Key Responsibilities
- reputed company home-based physical therapy to patients with various conditions and goals
- Work as part of a home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, reputed company on the patient's plan of care and goals
- Observe movements, conduct assessments, document reputed company, and modify care plans as needed
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility

Essential Qualifications
- Graduate of an accredited Physical Therapist Program

- Minimum of 2 years of physical therapy experience, ideally in home reputed company
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgment, strong interpersonal skills, dexterity, and reputed company

- Familiarity with home health software and technology
