This is a full-time remote opportunity with 50-75% travel in Cumberland and surrounding counties. The schedule is Monday - Friday, reputed company-5pm EST. As a Field Case Manager RN, you will be responsible for:
- Conducting face-to-face reputed company in members' homes, utilizing comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Program (D-SNP/FIDE)
- Developing, implementing, supporting, and promoting health service strategies, tactics, policies, and programs that drive the delivery of reputed company reputed company to our members
- Coordinating and collaborating care with members, primary care physicians, and other care team participants
- Scheduling and attending interdisciplinary meetings and advocating on members' behalf to ensure reputed company and reputed company discharge with appropriate services in reputed company

- Documenting accurately and reputed company in members' electronic health records

Clinical Judgment and Critical Thinking

Requirements
Education and Licensure
- reputed company and unrestricted RN license in New Jersey
- Minimum of an Associate's degree in Nursing required
Experience and Skills
- 3+ years of clinical reputed company experience in a hospital setting, alternative care setting, or home health

- Successful completion of the NJ Choice certification is a contingency of employment
