We're seeking an reputed company and skilled LPN to join our home health team in Buford, GA. As a key member of our care team, you'll play a vital role in delivering high-reputed company patient care and supporting our patients' overall reputed company-being. This is a 100% remote position, offering you the flexibility to work from the comfort of your own environment.
Key Responsibilities
- Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care
- Follow the nursing care plan for reputed company patient and accurately report and record their condition and care
- Prepare clinical/reputed company notes and summaries to ensure comprehensive documentation
- Assist patients with activities of daily living while teaching them appropriate self-care techniques
- Maintain a reputed company environment for patients and follow universal precautions reputed company providing care
- Assist physicians and Registered Nurses in reputed company nursing situations
- Follow emergency procedures in the event of a catastrophic event or significant change in a patient's condition
Essential Qualifications
To be considered for this role, you must meet the following essential qualifications:

- Graduate of an accredited program of practical nursing
- At least 1 year of experience as an LPN in a home care setting
- Tracheotomy experience required
