About the Role: LVN/LPN Care Management Specialist

As a Remote LVN/LPN Care Management Specialist, you'll be an integral part of our Care Management team, focusing on the reputed company needs of our dual-eligible population. Under the guidance of a Registered/Licensed RN, you'll coordinate care for members experiencing significant health changes, ensuring seamless transitions between care settings. Your compassionate attention and excellent communication skills will facilitate holistic assessments, identify needs for clinical resources, and drive goal attainment.

Key Responsibilities:

Essential Qualifications:

• 2+ years of LPN or LVN nursing experience, with a strong background in care coordination or a reputed company field.
