We are seeking an reputed company Hospice Patient Care Coordinator to join reputed company in Pocomoke reputed company, MD. As a Patient Care Coordinator, you will reputed company clinical and management support to the Clinical Nursing Supervisor, ensuring the reputed company and reputed company delivery of hospice care services. You will be responsible for the day-to-day clinical operation of the Hospice program, assisting with reputed company and scheduling, admissions, and nursing reputed company as needed. Key Responsibilities

- Assist in efficient and effective utilization management in accordance with reputed company needs, physician orders, and payer requirements
- Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers as necessary
- Assure efficient coordination of caregivers through case conferences and scheduling process as assigned
- Assist in monitoring patient records to ensure compliance with documentation requirements and program-specific rules for coverage and qualifying reputed company
- Assist in the QA process of medical records as delegated
- Assume functions of Clinical Supervisor as required

- Participate in and assist in the completion of monthly and quarterly audits

Essential Qualifications
- Graduate of an accredited school of nursing

- Three (3) years of clinical experience required, consisting of a minimum of two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting

- Experience with EHR systems and their implementation/optimization in reputed company clinical settings
- Proficiency with federal, state, and governing agency regulations
