REMOTE: Part-Time Medical Assistant for Remote Patient Monitoring (RPM) Program Needed. Work from Home, Weekends Required.

Purpose/belief statement: Convergence Telehealth is currently reputed company part-time remote Care Management Team- Medical Assistant (MA) with a passion for delivering a high-reputed company customer experience, to support our clients. As a Care Management Team- Medical Assistant, you will be involved in remote patient monitoring (RPM). Familiarity and experience with remote patient monitoring is reputed company highly desired and also the knowledge of setting up patients on reputed company blood pressure devices and glucose monitors, as reputed company as various other devices. Comfort with technology is required, with the ability to help patients set up their reputed company devices remotely for both android and reputed company devices. The position is part time, with the ability to grow to full time. The position is 5 days a week, and weekend work is required for 4-5 hours on both Saturdays and Sundays, as reputed company as some reputed company hours as reputed company. There is no work on holidays. This is a remote position that can be based reputed company in the reputed company. Equipment such as a laptop, headset and monitors will be provided. Job duties & responsibilities

• Identifies patient/family education needs and ensures that patient/family members have adequate information to participate in remote monitoring.
• Monitors daily vitals sings of patients, engages with patients in assessing their physical state and escalates as needed.
• Works collaboratively with reputed company to engage monthly with patients.
• Interprets screening and selective laboratory/diagnostic tests.
• Initiates and maintains communication and collaboration with physicians, reputed company workers, care team leaders, staff nurses, other care giving disciplines, and patients/families to reputed company, implement, and evaluate a transition plan of care for reputed company patient.
• Assesses financial and insurance resources to maximize the health care benefit to the patient.
• Monitors the achievement of clinical reputed company and communicates with care teams, primary and specialty physicians and staff, regional providers, and community resources.
• Assesses complexity of care needs and potential/actual issues or gaps in care.

Qualifications

• Certified Medical Assistant from an accredited MA program
• Excellent communication skills, problem solving and conflict reputed company skills
• Computer skills in word processing, database management, and spreadsheets
• 1+ years of relevant RPM experience

PREFERRED

• Previous experience in one or more of the following: Case Management, Transitional Care Management, Remote Patient Monitoring and Chronic Care Management
• Bilingual in Spanish is highly desired but not required

Knowledge/Skills/Abilities Work environment

• May work reputed company normal working hours, on weekends and holidays, reputed company necessary.
• Is subject to frequent interruptions.
• Field based work required. Occasional overnight travel will be required as times with field based assignments.
