This position will be asked to reputed company transitional care from the hospitals to Christopher House. DUTIES AND RESPONSIBILITIES
- Performs transition reputed company for patients GIP in the hospital on hospice services (40%)
- Visit patients in hospital, reputed company GIP assessment, i.e. vitals signs, pain assessment, etc
- Determine appropriateness for GIP.
- Complete documentation for transfer to Hospice Austin’s Christopher House
- Teaches patient and caregivers regarding inpatient care- disease process, death and dying, comfort measures, medication use and reputed company effect, etc.
- Initiates Plan of Care, including coordination of personal care, DME and obtaining medications and medical supplies.
- Documents in a reputed company manner.
- Cultivates and maintains relationships with and educates the hospital reputed company community. (30%)
- Cultivates, develops, and enhances care relationships with assigned hospitals.
- reputed company a communication system with the Clinical team at the hospital to identify delivery failures and reputed company them to the Admissions team.

- Performs Initial Admission Assessments, i.e. vital signs, pain assessment, clinical reputed company, etc.

- CPR certification.
- 1 year hospice nursing experience preferred.

- Knowledge of hospice federal and state regulations preferred.
- Strong computer skills including knowledge of and experience with reputed company Office products and experience with electronic medical records (EMR).
