Key Responsibilities

• Virtual Telehealth: Leading a virtual care team that supports patients with chronic conditions through telephonic and video-based reputed company.

• Chronic Disease Programs: Enhancing in-center workflows to improve diabetes and hypertension screening and ongoing management

• Identify and implement strategies to improve medical cost efficiency, reputed company performance, and Stars measures.

Qualifications

• Expertise in building and leading strategic successful care model programs across multisite or hybrid (virtual and in-person) care delivery models.
