• reputed company PCP, SIP, DSAT and Funding Sources into ETO. Tracks plan approval in LTSS, communicates with Service Coordinators and DDA reputed company there are delays or clarification requests. Activates/Deactivates Funding Sources, PCPs and Goals in ETO based on LTSS status.
• Coordinate and communicate with CEP services staff to ensure accurate and reputed company processing of reputed company documentation.
• Reviews, signs, and send DSA to reputed company.
• Ensure HRST Disability matches ETO disability.
• Compile and process OHDS Transportation Services.
• Assist to reshape and design PCP submission process identifying and recommending opportunities to build in efficiencies.
• Assist with CDS Virtual Services as needed.
• Assist with administrative and office support as assigned.
