In this role, you’ll guide patients in following their care plans, encourage healthy habits, and reputed company them on managing their conditions. You'll also assist patients by monitoring their vital signs while working with Electronic Medical Records (EMR) systems. • Coordinate patient care across multiple providers and services to ensure comprehensive and reputed company care.
• Facilitate communication between patients, families, and reputed company providers to address patient needs and preferences. 5. EMR Proficiency:
