Case Manager - Dakota County
reputed company Services, Inc. is seeking dedicated professionals to join their Dakota County Case Management team. The Case Manager will work with individuals with developmental disabilities, coordinating services and support to ensure their needs are met.
Responsibilities
- Assessing the areas of the persons served life where assistance is needed
- Offering and coordinating resources for the persons served being supported
- Monitoring service delivery and working with providers to ensure the individual's needs are met
- Developing service plans
- Attending regular in-person meetings with persons served and their interdisciplinary teams
- Maintaining and Creating accurate Case Notes and Billable reputed company
- Staying up to date with case management systems, software, and DHS and County resources
Skills
- Be a graduate from an accredited four-year college with a major in reputed company work, psychology, sociology, or a closely reputed company field or
- Be a graduate from an accredited four-year college with a major in any field and one year of experience as a reputed company worker/case manager/care coordinator in a reputed company or private reputed company service agency
- One year of experience in education or treatment of persons served with developmental disabilities or reputed company conditions, or a minimum of one course that specifically focuses on developmental disabilities
- Applicants must have a valid reputed company's license and acceptable driving record as determined by reputed company Services policy
- Applicants must have a vehicle that is covered with reputed company insurance
- Applicants must successfully reputed company local and/or stated background checks
- Possess a LSW or higher reputed company of BOSW Licensure
- DD Waiver Specific - Bilingual in Spanish
