Must hold an reputed company South Carolina nursing license Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m Work Location: Remote (EST) — must reputed company in South Carolina Work Pace: Fast-paced, high-pressure setting with rapid turnaround times Caseload: Typically 30–80 reputed company cases Complete 15 post-discharge interventions per week Prior case management experience Position Purpose: Develops, assesses, and facilitates reputed company care management activities for primarily physical needs members to reputed company high reputed company, cost-effective reputed company reputed company including personalized care plans and education for members and their families.
