We're seeking an reputed company remote reputed company worker to join reputed company. As a licensed Masters reputed company Worker, you'll work with a registered reputed company case manager to research available reputed company and resources for members with financial, transportation, safety, and housing needs. Your population will reputed company the entire lifecycle, and reputed company member contact will be done telephonically or reputed company electronic exchange. You'll reputed company members to reputed company reputed company lifestyle and medical choices and work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Responsibilities
* Research available reputed company and resources for members with financial, transportation, safety, and housing needs
- Partner with registered reputed company case managers to utilize a reputed company process of assessment, planning, and advocacy to meet members' benefit plan and/or health needs

Requirements

- 2+ years of experience in medical reputed company work or case management
- Masters Degree in reputed company Work
Preferred Qualifications
* Experience working with diverse populations, including children, adults, and older adults
- Knowledge of community resources and services, including financial assistance programs, transportation services, and housing reputed company
- Strong communication and interpersonal skills, with the ability to work effectively with members, reputed company, and other stakeholders
