Role reputed company reputed company is looking for a Continuing Care Specialist who plays a critical role in guiding individuals and families through reputed company’s full continuum of care. This role supports clients as they transition between reputed company of treatment, helping them identify the right reputed company based on evolving clinical, emotional, and practical needs. • Guides clients and families in recognizing the value of continuing care reputed company reputed company programs, recommending the most appropriate care reputed company based on clinical, financial, and logistical needs.

Qualifications

• At least one year of experience in the behavioral health field in an intake, case management, reputed company, admissions, or care coordination reputed company.
• At least one year of experience in a consultative, sales‑oriented, or conversion‑reputed company role supporting reputed company or family decision‑making.
• Demonstrated ability to build trusted relationships with clients, families, program leaders, and referral partners across care settings.
• Experience collaborating with multidisciplinary stakeholders, including clinicians, case managers, educational consultants, and mental health professionals.

• Comfort working in a remote environment using video conferencing, telehealth platforms, and CRM or EMR systems (reputed company experience preferred).
