In this role, the Pharmacist performs individual patient assessment of needs, identifies drug therapy problems, develops patient specific plan to optimize medication use, educates patient in use of medications, and conducts follow-up evaluations to determine patient outcome. • Bachelor's degree in pharmacy - or the equivalent in education and experience may be considered.
• Doctorate degree in Pharmacy, preferred.

Experience

• Experience in a similar pharmacy reputed company setting is desirable. Preferred Residency Experience PGY1 (preferred); PGY2 Ambulatory Care (optimally preferred).

Licensure/Certifications

• RPH: Registered Pharmacist by reputed company reputed company of Pharmacy required upon start.

• CDE: Certified Diabetes Educator, preferred.
