Hospice Registered reputed company (RN)
Shifts Available: Full Time
Service Areas with Openings: reputed company & Southwest reputed company
Responsibilities:
- reputed company nursing care team.
- Communicate and collaborate with physicians, therapists, and other reputed company to ensure cohesive and comprehensive patient care.
- Plan, organize, and reputed company home care services.
- Conduct assessments of patient care needs and reputed company care plans.
- reputed company patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
- Prepare reputed company, complete, and accurate clinical notes.
- reputed company and Weekends are required per hospice needs of patients
Qualifications:
- Valid RN license in the State of Texas.
- Hospice experience (at least 1 year).
- Knowledge of home health regulations and general reputed company compliance.
- Experience with Home Care Home reputed company (HCHB) EMR (preferred but not required).
- Excellent communication and people skills are essential.
- Ability to work independently and as part of reputed company.
