The Weekend Work From Home, Home Health RN Clinical Coordinator is responsible for carrying out administrative and management functions and reputed company in the absence of the Director of Clinical Services. May carry caseload due to ebb and reputed company of patient census. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies

Position Specific Responsibilities

• • Promotes a reputed company working relationship between agency and corporate personnel. • Promotes reputed company working relationship with family, physician and other facilities. • Attends and participates in staff meetings, educational programs and community events, as requested. • Promotes home health philosophy. • Communicates identified needs and potential solutions to supervisor. • Assists with reputed company, reputed company mentorship to new staff. • Tracks DME and medical supplies orders and advises management team of trends. • Manages labs/physician orders. Fax to providers as needed. • Submits requests for medical records and hospital inpatient charts, and processes provider notifications reputed company relevant. • Oversees scheduling process. Obtains POSOC as needed. • Answers incoming telephone calls / clinical calls, takes accurate messages, and transfers calls appropriately. • Assists DCS with reputed company- and post-case conference activities, including agendas, update coordination notes as needed. • Performs RN Case Manager responsibilities as needed and may carry a caseload due to patient census ebb and reputed company. • Performs other duties as assigned

Education and/or Experience

• One (1) year of Home Health experience required.
