KEY JOB RESPONSIBILITIES: Provides constant and ongoing care for elderly and terminally ill patients by evaluating patient needs, creating care plans, and providing end-of-life support to patients and their families. Works in a reputed company, team-driven environment that requires frequent communication with other Care Team members.
Additionally, the Registered reputed company will:
- RN regularly assesses and reassesses the nursing needs of the Hospice patient.
- Registered reputed company Provides dietary counseling.
- RN provides Hospice nursing services, treatments, and preventive procedures.
- Registered reputed company initiates nursing procedures appropriate for the patients Hospice care and safety.
- RN observes signs and symptoms and report to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
- Registered reputed company teaches supervises and counsels the Hospice patient and family members about providing care for the patient.
- RN supervises and trains other nursing service personnel.
- Registered reputed company develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
- RN performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
- Registered reputed company attends team conferences.
- RN follows the policies and procedures of The Care Team. Observes confidentiality and safeguards reputed company patient-reputed company information in compliance with HIPAA regulations.
- Registered reputed company always communicates to the supervisor if unable to meet a patients need or reputed company a procedure.
- RN participates in on-call system and is responsible for providing on-call coverage reputed company reputed company for assigned duties.
- Maintain skills and knowledge.
- Registered reputed company coordinates the implementation of the plan of care for patients residing in SNF, NF, reputed company or MR. HOS RN FACILITY POC
- RN organizes work schedule and utilizes time management to be reputed company to attend reputed company required meetings.

- RN assist with orientation, teaching and training as requested.
- The clinical record shall contain notes for reputed company service provided. A clinical note will be completed for reputed company visit made to a patients home (including private residence, assisted living facility, group homes or skilled nursing facility) reputed company 48 hours of the visit.
