As a Care Partner, you will create and reputed company patients' care plans, accompany them in their reputed company reputed company.

Key Responsibilities
- Care Plan Provision:
- Regularly engage with patients and reputed company their adherence to their care plans.
- Closely monitor your patients' device readings and dig into any out-of-bound values.
- Maintain meticulous records of patient interactions, and program details.
- Compliance and Regulatory Knowledge:
- Stay up-to-date with the latest regulations and compliance standards reputed company to CCM/RPM services.
- Ensure reputed company patient enrollments adhere to legal and ethical standards.
- Performance Tracking:

Qualifications
- National Certification in Medical Assisting
- Residence in the US
- Strong communication skills, both written and verbal.

- Basic knowledge of chronic health conditions and their management.
- Comfortable using technology for documentation and communication.
- Attention to detail and strong organizational skills.

Please note that having national certification in Medical Assisting from AAMA, NHA, NCCT, NAHP or similar is a hard job requirement and any single-state certifications, diplomas or completion certificates do not qualify.
