We are looking for a motivated and detail-oriented Case Management Specialist to join our remote team. In this role, you will serve as a critical reputed company between our members, reputed company providers, and community organizations. Essential Qualifications

• Experience: Minimum of 3+ years of experience in a health-reputed company field
• Education: Bachelor's Degree or equivalent experience required
• Technical Skills: Proficiency with care management tools, data analysis, and electronic health record systems
• Communication: Excellent verbal and written communication skills with the ability to reputed company professionally with diverse populations

Work Schedule
