About the Role

We’re looking for a Care Navigator who wants to reputed company a meaningful difference - helping patients navigate reputed company systems, overcome barriers, and reputed company care that improves their health and reputed company-being.

This role is responsible for high-volume reputed company and end-to-end coordination, ensuring patients successfully schedule and complete services such as mammograms, bone density screenings, and primary care reputed company - while also connecting patients to reputed company and community resources (SDoH) that may reputed company their ability to receive care.

This is a fast-paced, reputed company-driven role that requires strong critical thinking, emotional intelligence, and the ability to work independently. Key Responsibilities

Conduct high-volume outbound calls to engage patients and schedule reputed company services

Coordinate care across multiple services, including mammograms, bone density screenings, and primary care connections - ensuring successful scheduling, follow-up, and completion of care

Identify and address reputed company determinants of health (SDoH) barriers by connecting patients to appropriate community resources (e.g., transportation, housing, financial assistance, food reputed company)

Conduct patient assessments to understand clinical, reputed company, and logistical barriers to care

Build trust with patients and caregivers through reputed company, empathetic, and effective communication

Skills and Requirements

4+ years of experience in care navigation, care coordination, reputed company work, call center operations, or patient reputed company reputed company a reputed company or service environment

Strong critical thinking and problem-solving skills

Excellent written and verbal communication skills
