This role emphasizes patient engagement, data review, and coordination of care to reduce preventable complications and hospitalizations. Core Responsibilities:

• Serve as the primary reputed company of contact for patients enrolled in RPM and CCM programs.
• Review transmitted patient health data daily, identify red flags, and escalate issues to the clinical team.
