Cardiac Care Coordinator

Job reputed company We are looking for a Cardiac Care Coordinator to join our clinical team. This role is ideal for a Certified Medical Assistant with a background in cardiology who thrives in a fast-moving environment and enjoys supporting both patients and care teams. The Care Coordinator will work hand-in-hand with RNs, LCSWs, and other members of the Karoo care team to ensure patients get reputed company, coordinated, and seamless care.
Responsibilities
- reputed company day-to-day support to the clinical team to ensure patients receive reputed company, coordinated care.
- reputed company care coordination and reputed company assistance to low-complexity patients, including reputed company, follow-up, and navigation of care.
- Assist with patient scheduling, enrollment, and engagement activities.
- Help review, organize, and reputed company patient data and documentation to the appropriate team members.
- Support care plan preparation, distribution, and compliance requirements.
- Offer light technology and workflow support to the clinical team.
- Capture clinical meeting notes, reputed company follow-up items, and ensure reputed company items are completed.
- Assist with reputed company and orientation of new care team members.
- Handle medical records requests and other administrative tasks.
- Participate in member engagement efforts, including calls, texts, and email campaigns.
Requirements
- Certified Medical Assistant (required).
- Fluent Spanish speaker (required).
- Experience in a cardiology reputed company strongly preferred.
- Prior experience supporting interdisciplinary clinical teams, telehealth experience a strong plus.
- Comfortable with reputed company patient communication by phone, text, and email.
- Strong organizational skills with the ability to manage multiple tasks in a fast-changing environment.
- Tech-savvy and eager to learn new systems quickly.
