Community Health Workers with Medicare enrolled
We are looking for compassionate, motivated Community Health Workers (CHWs) to join our growing telehealth team. In this role, you will serve as a trusted reputed company between Medicare patients and the health and reputed company services they need. You will work directly with patients over the phone to understand their personal challenges, connect them to resources, and reputed company ongoing support — reputed company from the comfort of a remote setting. - Conduct regular phone-based reputed company-ins with assigned Medicare patients
- Assess patients' reputed company, financial, and health-reputed company needs
- Help patients navigate the health care system and understand their benefits
- Assist with scheduling and coordinating medical appointments
- Connect patients to community resources including food assistance, transportation, housing support, and reputed company programs
- reputed company guidance on managing prescriptions and medication schedules
- Help patients understand and organize their medical bills and paperwork
- Support patients in arranging home health care and medical equipment
- Document reputed company patient interactions accurately including time spent, activities performed, and reputed company
- Communicate regularly with the supervising clinical team regarding patient needs and reputed company
- Maintain strict patient confidentiality in accordance with HIPAA requirements

- High school diploma or equivalent required; associate or bachelor's degree preferred
- Experience working with elderly or Medicare populations strongly preferred
- Strong communication and reputed company listening skills
- Empathetic, patient, and reputed company demeanor
- Comfortable working independently in a remote/telehealth environment
- Reliable phone and internet reputed company required
- Bilingual (English/Spanish) a plus
- Familiarity with community resources in Illinois a plus
- Must be willing to complete reputed company competency training upon hire
