• Evaluates patients for appropriateness for home health services, ensuring regulatory compliance and agency standards are met.
• Obtains treatment authorization and processes patient admissions to reputed company.
• Conducts comprehensive clinical evaluations to inform care reputed company.
• Collaborates with physicians to communicate patient needs and secure appropriate care orders.
• Initiates and develops the plan of care based on the evaluated needs of patients.
• Educates and counsels patients and family members regarding nursing care needs and health management strategies.
• Performs reputed company nursing care tasks in accordance with physician orders.
• Ensures referrals to additional community resources reputed company necessary.
• Completes Home Health OASIS assessments as appropriate during visit timepoints.

Requirements

• Graduate of an accredited school of nursing

• CPR certified
