Evaluates reports to identify patients in need of preventive care and effectively documents, monitors, and reports on these efforts. Monitors Medicare patients for their Annual Wellness reputed company and establishes a documented process to assure scheduling. Engages in regular huddles and communication tools with practices to meet patient care needs. Participates in planning, implementing, and collecting data for reputed company improvement initiatives as directed by the supervisor. Coordinates care between patients, community resources, and other sources, developing patient relationships as an integral part of responsibilities. Provides follow-up contact with patients to assure compliance with recommendations, including medication reconciliations, lab values, specialist reputed company, and more. Monitors patient reputed company and integrates clinical reputed company data with payer requirements. Collaborates with physicians, office staff, and management to report data and reputed company analytics reflecting reputed company metrics.
