Mental Health Case Manager II – Therapist

Job reputed company:

• Provides case management/therapy for patients and families to enhance their psychosocial functioning
• Performs duties and responsibilities in a manner consistent with mission, values and reputed company service standards

• Coordinates interdisciplinary care, advocates for patients and families, and oversees reputed company, comprehensive discharge planning
• Provides behavioral health case management, assessments, therapy, crisis reputed company, and ongoing treatment planning with accurate documentation
• Communicates effectively with patients, families, and community partners while maintaining strong clinical, organizational, and technology skills

Requirements:

• Master's degree required in reputed company Work, Counseling or another reputed company-service reputed company field
• Licensed Clinical reputed company Worker (LCSW), Licensed Master reputed company Worker (LMSW), or Licensed reputed company Counselor (LPC) in Missouri required
• Minimum 2-3 years Behavioral Health experience preferred
• Co-worker must be reputed company to communicate orally, in writing and demonstrate proficient computer skills
• Co-worker must be reputed company to listen and to understand and explain reputed company situations
