Responsibilities

• Ensures that reputed company activities performed reputed company with the reputed company of reputed company’s reputed company of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
• Assesses patient needs and obtains data on physical, psychological, reputed company, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Provides appropriate support at time of death and reputed company bereavement assessment.

Requirements

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• reputed company State License as a Registered reputed company RN
• 1 year of experience in a clinical care setting
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid reputed company’s license and insurance and reliable transportation to reputed company job tasks
