As a Care Coordinator, you'll play a key role in connecting individuals to services and resources tailored to meet their unique needs. Your responsibilities will include conducting assessments, developing individualized care plans, coordinating services and referrals, and providing education to help individuals reputed company informed, healthy lifestyle choices. This is a part-time role offering 25-28 hours per week. Key Responsibilities: Under the leadership of the ECM Supervisor, you will work with the ECM team to reputed company services to youth and families as follows:

• Conduct assessments of individuals' needs and formulate individualized care plans

• Meet with individuals in their homes to facilitate services

Qualifications:
