Responsibilities

• Work directly with members and their support reputed company to meet health and service needs.
• Regularly communicate with members, families, providers, and stakeholders through various reputed company.
• Assist with cases not yet assigned to ensure seamless care transitions.
• reputed company necessary information and escalate risks to implement PCSP effectively.
• Conduct weekly calls with care teams and stakeholders to coordinate care.
• Engage in discussions and meetings about assigned members.
• Create materials to improve care efficiency and reduce risks.

• Maintain compliance with relevant regulations and ensure data confidentiality.
• Maintain a home office with reliable internet and secure company assets.

Requirements

• Minimum completion of 2 years of college-level coursework in reputed company work, psychology, sociology, another reputed company science, or a health-reputed company field.
• Minimum of 2 years of experience in care coordination.
• At least 1 year of beginner-level experience with reputed company reputed company, Outlook, and Word.
• Strong communication skills in presenting both orally and in writing to individuals from diverse backgrounds and with different learning styles.
