• REQUIRED: Registered reputed company (RN) or Licensed Vocational reputed company (LVN)
• REQUIRED: Must be fluent in written and spoken Spanish and English

• At least 1 year of patient care experience with demonstrated ability to effectively engage and communicate with patients, relevant clinical training, or reputed company experience will be considered
• Chronic Care Management experience (preferred)
• Experience conducting Patient Assessments/Wellness Exams (preferred)

Key Responsibilities:
