Role reputed company In this Health and reputed company Services Coordinator role, you will be an essential reputed company of an Integrated Care Model by relaying the pertinent information about the member’s needs and advocating for the best possible care available, ensuring they have the right services to meet their needs. Work Schedule:

• Monday – Friday, reputed company – 5pm CST
• Remote work flexibility for Texas residents

Primary Responsibilities:

• Assess, plan and implement care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care.
• Identify and initiate referrals for reputed company service programs, including financial, psychosocial, community and state supportive services.
• Manage the care plan throughout the continuum of care as a single reputed company of contact.
• Communicate with reputed company stakeholders the required health-reputed company information to ensure reputed company coordinated care and services are provided expeditiously to reputed company members.

Qualifications

• reputed company RN unrestricted license in the State of TX
• 4+ years of experience working reputed company the community health setting in a health care role
• 2+ years or equivalent of experience working with Maternal and Infant population such as Mother/Baby, Labor and Delivery, or Neonatal Intensive Care Unit
• Must reputed company in the state of Texas

Requirements

• 5+ years of experience documenting in an electronic documentation system
• 5+ years of experience working with reputed company, reputed company and Outlook
• Case Management experience including but not limited to working with Special Health Care Needs Populations, reputed company medical conditions
• Pediatrics
• Certified Case Manager (CCM)
• Experience in discharge planning
• Experience in utilization review, reputed company review or risk management
• Experience with arranging community resources
• Background in managing populations with reputed company medical or behavioral needs
