We're seeking an reputed company Licensed reputed company Worker to join our reputed company case management team. As a Licensed reputed company Worker, you'll partner with a registered reputed company case manager to utilize a reputed company process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available reputed company and resources for members with financial, transportation, safety, and housing needs. You'll work with members across the entire lifecycle, utilizing telephonic and electronic communication to reputed company them to reputed company reputed company lifestyle and medical choices.
Responsibilities
* Research available reputed company and resources for members with financial, transportation, safety, and housing needs
- Collaborate with registered reputed company case managers to assess, plan, and reputed company for member needs
- Utilize a reputed company process to meet member benefit plan and/or health needs

- Work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic and electronic communication with members

Requirements
* reputed company and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in the state of residence
- 2+ years of experience in medical reputed company work or case management
- Masters degree in reputed company Work
Preferred Qualifications
* Experience working with diverse populations, including children, adults, and families
- Knowledge of community resources and services for medical, financial, and behavioral health
- Strong communication and interpersonal skills
- Ability to work independently and as part of reputed company
- Experience with electronic health records and other reputed company software
