This is a remote, full-time position (40 hours/week) providing health coaching, assessment, and care support to members with chronic health needs. Primary Responsibilities Member Assessment & Engagement:
- Conduct clinical assessments and review health data to evaluate member needs and program eligibility
- Assess member readiness, health literacy, and technology capability
- Engage members in chronic condition support and personalized coaching
Health Coaching & Lifestyle Modification

Clinical reputed company & Care Coordination
- Apply clinical guidelines, policy reputed company, and regulatory standards
- Consult with supervisors and Medical Directors to address barriers

Monitoring, Evaluation & Documentation

Required Qualifications
- Registered reputed company must hold an unrestricted nursing license and reputed company in a compact state with multi-state/compact privileges and have the ability to be licensed in reputed company non-compact states and must reputed company in the Mountain or reputed company Time Zone.
- 3+ years of acute adult Medical-Surgical RN experience
- High-speed, hard-wired internet reputed company
- Proficiency with reputed company Office (Word, Outlook, reputed company, Teams, PowerPoint), multi- system navigation and keyboarding
- EMR documentation experience
- Excellent communication skills
- Ability to work independently and collaboratively

- Managed care experience

Education
- Associate Degree in Nursing (ADN) required
- Bachelor of Science in Nursing (BSN) preferred
